ICMA-VIII funding request form

There are two or three parts to the request for funding

  1. Complete this funding request form
  2. Email a copy of your CV to ICMA2022@louisiana.edu
  3. If you are a student, ask your advisor to email a letter of recommendation to ICMA2022@louisiana.edu
    Letters of recommendation must be received by 1 September 2022.

Please enter your affiliation (College/University, Company, Organization, other). Include organization name and department or division name as appropriate.
Please enter the title of your talk or poster presentation.
Please indicate your citizenship status by checking the appropriate choice. If you are not a US citizen or permanent resident, please check other and provide your country of residence and visa status on the next line.

Please indicate the number of years you have been enrolled in your graduate program or the year in which you received your graduate degree. Please enter zero if you are an undergraduate student.
Please indicate the amount you are requesting for lodging, not to exceed $115 ($78 for a student) per night for up to three nights (including taxes and fees).
Students are expected to share a room. If you have a preferred roommate, please enter their name and email address here.
Please indicate the amount you are requesting for travel, not to exceed $640.
Please indicate your expected mode of transportation to Lafayette and the city you are departing from.
Please enter your arrival date.
Please enter your departure date.
Please indicate whether you have other sources of travel funding. If you do have another source of funding please describe the source and amount of this funding in the space on the next line.
